I agree that the information I have given on this form is correct to the best of my knowledge. It will be held on the MIT database, for administration purposes and to help me find employment and/or a placement on an Apprenticeship. I give my permission for my image and/or voice to be recorded and that this information may be shared with any other organisation in association with MIT.

 

I, the applicant, agree that MITSkills and its partner companies can access my PLR record through the Learner Record Service website for the purpose of acquiring my education history. This information will be used, in conjunction with other documents, to help determine my eligibility for the course I have applied for. This information can be used to exempt me from certain mandatory aims attached to my course, such as functional skills. The information contained in my PLR check may also be made available to my employer and the ESFA. Please note that the Learner Record Service is not always a complete history, and you may be asked to present certificates as well. I can confirm that by signing this document I have read and agree to the above and the ESFA privacy notice.
